The post holder will work as part of a Community Nursing Team to provide nursing care without direct supervision for patients registered with GPs within Midlands Partnership NHS Foundation Trust, covering Rugeley & Great Haywood PCN. The post holder will participate in the ongoing assessment, planning, implementation and evaluation of programmes of care adhering to policies and procedures. Strict confidentiality regarding all information concerning patients, staff and sensitive Trust business will be maintained at all times, in accordance with Caldicott Guidance and Data Protection Act 1998.
