The District Nurse is responsible and accountable for assessing, planning and implementing programmes of care. These programmes will be developed in partnership with patients, family, whanau/carers and significant others, and will be evidence based taking into consideration lifestyle, race, gender, cultural background and factors which impact on physical, mental or emotional health.
The District Nurse works in partnership with patients and their families/whanau and collaborates with the multidisciplinary team in both primary and secondary health settings, recognising and supporting the delivery of Health New Zealand Te Whatu Ora Lakes vision and demonstrating values in the workplace.
