Position Overview
Summary:
The Discharge Planner is responsible for negotiating, procuring, and coordinating appropriate services and resources needed by patients after the acute inpatient phase. This role is capable of performing non-complex discharge planning tasks, such as returning patients to nursing homes, ordering durable medical equipment (DME), coordinating Home Health services, and faxing referrals to placements.
