This permanent, full-time position is dedicated to fostering impactful relationships and creating individualized service plans based on the assessed needs of each youth. You'll actively engage with clients, families, and community activities, promoting comprehensive support and cultural safety in service delivery.
Key Responsibilities:
• Enhance youth independence through targeted mentorship
• Construct tailored service plans addressing client strengths
• Engage youths in community recreational activities
• Conduct group-based training to develop essential skills
• Maintain detailed clinical and case management records
Requirements:
• BSW or related degree is highly preferred
• A minimum of 3 years working with CYSN clients
