Position: Community Reablement Support Worker
Location: Salisbury, Wiltshire
Salary: £29,979 to £31,458 per annum inclusive of 10% unsocial hours enhancement
Hours: Full time, 37 hours per week
Contract: Permanent

An opportunity has arisen to join a community-based support team helping people safely transition from hospital back into their own homes.
