Community Mental Health Triage Nurse

An exciting opportunity has arisen within the South Kensington Community Health Hub for a triage and assessment mental health practitioner.

The role is to screen referrals, undertake specialist assessments and provide brief follow‑up or facilitate an alternative service provision for people referred to the team. It is a front‑line post involving face‑to‑face work with service users and their carers. Post holders must be able to make clear and autonomous decisions based on the complexity of presentations due to the person's health and social care needs.
