Location: Community-based Minehead Medical Centre & patient homes

Hours: Part‑time 20 hours available (Full‑time 40 hours)

Minehead Medical Centre is seeking an experienced Community Matron to join our primary care team.

If You

• Are a registered Nurse and a fully‑qualified Independent Provider.
• Enjoy building strong, trusting relationships with patients and their families.
• Desirably have experience supporting older adults including frailty, geriatrics, care homes or complex long‑term conditions.
• Can deliver proactive clinical care, polypharmacy review, and anticipatory care planning.
