UPMC Community Care Behavioral Health is hiring a flex full-time Community Health Worker (CHW). This role will work 28 hours/week between 8:30 am - 5 pm Monday through Friday. There is flexibility in the schedule with the preference that the CHW works some hours each day and has the flexibility to adjust hours to meet members needs. This position predominantly travels throughout York and Adams Counties with some time spent working remotely.

The Community Health Care Worker (CHW) will work directly with individuals receiving services, family members, providers and other stakeholders, at the direction of a supervisor. This position requires the candidate have close understanding of the community they serve.
