Job Function:
Shift and schedule: Monday-Friday | 9am to 5pm w/ flexibility as needed to accommodate client availability.
Conduct timely outreach to referred clients and complete Health-Related Social Needs (HRSN) screenings to identify unmet medical, behavioral, and social needs.Engage clients in community settings, providing education, support, and ongoing care navigation.Connect clients and household members to appropriate medical, mental health, substance use, and social service resources.Support individuals with mental health conditions, substance use disorders, and developmental disabilities using a person-centered approach.Maintain strict confidentiality while exercising independent judgment in handling sensitive information.Collaborate with community partners and interdisciplinary teams to ensure co...
