A Medical Center is seeking a passionate and dedicated Community Health Advocateto join our Community Health Work & Sustainable Outreach and Navigation (CHW SON) team. This position is grant-funded and will play a vital role for outreach and engagement to provide direct referrals to services for people who were the most affected by the pandemic. The goal is to further reduce health disparities by addressing social determinants of health, offering early intervention, and promoting health education.

Responsibilities:

● Proactively reach out to community members and their families to provide information and support.

● Conduct initial assessments and data collection to understand individual needs and connect them to community resources.
