**Required Qualifications:**



+ Graduation from an accredited school of pharmacy with didactic coursework and experiential rotations in pharmaceutical care.

+ Upon hire: Washington Pharmacist License



**Preferred Qualifications:**



+ PharmD

+ Post-graduate degree in pharmacy, Or

+ Completion of a post-graduate pharmacy residency in geriatrics.
