**Description**



The Clinical Pharmacist is responsible for assessing patient specific medications and clinical data in order to evaluate & monitor for safety, efficacy and pharmaco-economic optimization opportunities. In this role, the pharmacist may administer medications, monitors clinical data, educates patients, and provides general drug information to the healthcare team.

**Required Qualifications:**



+ Bachelor's Degree - Pharmacy from an accredited School of Pharmacy _-OR-_
