Summary The St. Louis VA Healthcare System is seeking a Clinical Pharmacist that facilitates resolutions of Veteran concerns. They will be responsible for interpretation of provider medical orders, prescription order/entry/completion, patient compliance monitoring, patient education, provider drug information consultations, and patient medication therapy monitoring. Responsibilities Handles routine medication-related activities. Makes recommendations to clinical staff regarding medication therapy. Documents recommendations and interventions.
