**Full-time | hours per week** A busy and well-established primary care practice in MK40 is seeking an experienced **Clinical Pharmacist** to join its multi-disciplinary team on a permanent basis. This role will support safe prescribing, medicines optimisation and direct patient care. * * * **Key Responsibilities**
------------------------ * Conduct clinical medication reviews (face-to-face, phone or notes-based)
* Provide specialist medicines advice to clinicians and patients
* Lead medicines optimisation and safety initiatives
* Review discharge/specialist treatment letters and action medication changes
* Support public health activities ( flu and routine immunisations)
* Run medicines-focused clinics ( anticoagulant monitoring)
* Support dispensary team when needed
