Position Overview
Reporting to the Assistant Director, Living in Place, the Clinical Coordinator, Living in Place, provides clinical support to Queens based, Aging Services programs at Breaking Ground using a Critical Time Intervention (CTI)-informed model; providing time-limited services focused on housing retention, resource development, and community integration intended to support older adults with aging safely in the community, and provides information and referral services; benefits application and navigation assistance; resource development support; and community integration programming. The case management for Aging Services is typically a 3–6 month engagement and on a referral basis.
The Clinical Coordinator maintains a caseload of 30-40 clients and serves as a member of the Tenant 360 management team.
