**Job Summary:**

Key Responsibilities:

+ Function as an interdisciplinary member of the patient care teams to support all clinical pharmacy patient care activities in the Seniors Clinic.
+ Establish collaborative working relationships with clinic providers and staff to become a valued and contributing team member.
