Role and Responsibilities


The Client Relationship Specialist is a NYS license practical nurse who is responsible for developing and maintaining client relations and intake of direct referrals made to HCR .


Essential Functions


+ Cultivate relationships with our physicians and hospitals to facilitate the best Home Care solutions for their patients.

+ Monitor HCR RightFax folders, label faxes and attach electronic documents to patient record in Home Care Home Base (HCHB).

+ Monitor Intake Team email.

+ Produce timely, accurate data entry.

+ Responsible for entering orthopedic prescreens into HCHB according to agency protocol.

+ Provide phone support to the department and triage of calls to Intake staff.
