CHMC –RN Case Manager – Full Time-Day Shift – Req 9467

Pay Range: $37.63-$49.86*

We are seeking a highly organized and clinical RN Case Manager to oversee the care of complex patients. In this role, you will act as a central coordinator, negotiating and managing resources to ensure high-quality, cost-effective outcomes from admission through discharge.

Key Responsibilities
+ Care Coordination: Manage complex patient cases by collaborating with interdisciplinary teams both inside and outside the organization.
+ Strategic Planning: Merge clinical needs with financial processes, interpreting Medicare, Medicaid, and private insurance benefits to facilitate care.
+ Quality Improvement: Participate in performance evaluation processes to enhance patient care across the entire continuum.
