Position Overview

Job overview
The post holder is expected to assess, plan, implement and evaluate care of the child/young person (CYP) in the community setting, children's centre and acute paediatric ward.  The CYP will have a  nursing need, categorised into four main groups.

CYP with acute and short term conditions.

CYP with long term conditions. 

CYP with disabilities and complex conditions. 

CYP with life limiting and life threatening conditions.

The post holder will facilitate and provide safe and effective care working in partnership with the CYP, family/caregivers and the multidisciplinary team, to ensure holistic and family centred care in the most appropriate setting supported by a senior community children's nurse. 

The post holder will work autonomously as well as part of a team/integrated agency approach in provision of high quality, evidence based care.
