**Job Description**

Will be responsible for managing and coordinating comprehensive care for clients within the CCBHC framework. This role involves working collaboratively with interdisciplinary care teams to ensure clients receive the necessary services and support to improve their health outcomes and quality of life. The care coordinator provides outreach and advocacy services to persons entering the CCBHC.
