Position Overview
The Caseworker works within the Clinical Department, with the Aging and Community Living Supports (ACLS) programs. The caseworker conducts in-home ongoing assessments, and phone consultations to assist in identifying participant needs and arranging services and supports to meet these needs which, facilitates program participants living with dignity and independence in the community. The Caseworker coordinates services provides ongoing follow-up to program participants and completes various tasks as needed to support team member functions. Job Duties:
+ Conducting ongoing in-home reassessments and phone consultations with participants and/or their representatives, reviewing changes in areas such as health, physical function, environment, finances, cognition/mood, and informal support.
