FULL-TIME Monday to Friday 8am-4pm (with flexibility to the needs of the home)

Position Summary

The Case Manager plays a key role in supporting mothers and newborns living in the home. This position works directly with residents to develop and implement individualized care plans that promote parenting capacity, stability, and connection to community supports. The Case Manager provides advocacy, coordination of services, and culturally safe support while working collaboratively with families, Elders, community resources, and child welfare partners.

Key Responsibilities

• Conduct comprehensive intake and ongoing assessments for mothers and newborns.
• Develop individualized case plans focused on family preservation, parenting support, and wellness.
