The Embark Care Case manager acts as the principal agent in the supervision of field staff and overall client care to ensure that daily operations are performed at a high satisfactory level. The purpose of this position is to enhance the quality of service provided, maximize satisfaction, increase retention and promote cost effectiveness. This position will be accountable for overseeing the full client care cycle including: assessing, planning, implementing, monitoring, hiring of qualified personnel, scheduling, and evaluating actions required to meet the clientβs needs.
Qualifications:
Required Graduate of accredited High School. Required Minimum of two (2) yearsβ experience in health or home care related positions.
