+ Maintain accurate case files and documentation
+ Advocate for client needs with community partners
+ Monitor progress and update care or support plans as needed
Requirements
Requirements:
+ Bachelor’s degree in social work, human services, or related field preferred
+ Experience in case management, social services, or community programs
+ Strong interpersonal and crisis-management skills
+ Knowledge of local resources and referral processes
+ Ability to maintain confidentiality and accurate documentation
