**Description**

**Job Purpose:** To facilitate the improved wellbeing of individuals with severe mental illness and/or substance use disorders. The Case Manager will work collaboratively with an integrated treatment team to provide clinical direction and support to a pool of clients. Case managers will develop and implement a treatment plan, which will include evolving goals and client-centered interventions.
