Description

Comunilife Inc. is looking for a Case Manager for its congregate program in the Bronx, La Central. The Case Manager’s main responsibility is to provide case management services for a caseload of clients and their family members. The Case Manager is responsible for full coordination of services for all clients assigned; ensure that all assessments are completed and all required/needed services are rendered or arranged based on service plan(s).
