**Position Summary:** The case manager will provide support and service linkages to residents within the program. Coordinate services and ensure quality care to the residents.

**Essential Responsibilities:**

1. Responsible for the operation of the Supported Housing program and assigned caseload. Drive self and/or drive agency vehicle throughout boroughs to visit apartments routinely, to assess, observe and evaluate physical plan and residents functioning abilities and needs.
