Are you passionate about making a difference through case management? We are seeking a dedicated and motivated Case Management Officer to manage cases towards closure and transfer, ensuring smooth coordination with clients, community partners, and professionals.
Key Responsibilities:
- Manage cases and work closely with clients and professionals, such as community agents, to ensure case closure and transfer.
- Coordinate with officers, supervisors, and agencies for case triage and/or introductory meetings with clients and agencies.
- Provide administrative support to ensure the proper closure and transfer of cases to community partners and professionals.
- Draft and submit emails, referral forms, and closure reports to relevant agencies.
- Update data and upload necessary documents in the case management system.
- Assist with other tasks as assigned by the supervisor.
