**Description**



The Care Manager RN is responsible for managing patient populations with chronic illness and/or multiple co-morbidities. This position works in coordination with multiple PCPs and serves as the central link to an identified population of patients.

**Required Qualifications:**



+ Coursework/Training: Graduation from an approved, accredited school of Nursing.

+ Upon hire: Washington Registered Nurse License

+ 2 years of work experience in an RN role.
