JOB DESCRIPTION

Highlights of the experience needed to fill these positions follows. All four of the opportunities are supporting our Medicare/Medicaid members. Please read the details below prior to applying:

Two openings for RNs with a behavioral health background to work telephonically.

+ Chosen RNs will have a caseload focused on members with behavioral health diagnosis.
+ Members will have SUD and/or SMI diagnosis who have been hospitalized.
+ Care Management will include co-managing their care with the field care manager.
+ Candidates must have an active RN license in TX and reside in the state.

There are two field care manager openings as well and local travel is required.

+ The field openings are in San Antonio and the northwestern part of Harris County
+ These RNs will meet with members in their homes completing assessments that will determine the types of services they are eligible to receive.
+ Mileage is reimbursed...
