The Always Best Care's Care Coordinator (CC) position is responsible for coordinating the care delivery for Always Best Care home care/non-medical clients. The CC is responsible for teaching, coaching, and supporting the Direct Care Worker (DCW) to ensure the client’s specific care delivery is provided. The goal of this position is to enhance the client’s quality of care by coordinating the care delivery process in a seamless, safe, and effective manner. The CC will maintain and oversee client’s care coordination by developing relationships with internal and external customers to better meet and anticipate client’s needs. The coordination of care begins at the Start of Care (SOC) until the discharge process from Always Best Care’s services is completed.
