**Position Purpose**

The Care Coordinator is responsible for coordinating services, support, and resources to ensure individuals receive high‑quality, person‑centered care. This role serves as a key liaison among individuals, internal teams, and external providers to promote continuity of care, safety, and positive outcomes.

+ Serve as the primary point of contact for care-related communication and problem-solving
