Position Overview
Job Description
To provide basic care coordination, discharge planning, monitor/evaluate and education for clients and their families on the services offered by NTUC Health Co-Operative Ltd.
You will need to:
(a) Information and Referral (I & R)
Manage various referral sources such as emails, phone calls and walk-ins in order to link clients to appropriate services by identifying and accessing their needs. (b) Service Coordination
Administer the coordination of service requested by clients. To ensure proper documentation on the systems and paper filing are completed on a timely basis. (c) Case Management
Conduct regular home visits and telephonic follow-ups on a monthly basis to engage and build rapport with clients and their caregivers, and obtain feedback on the services rendered.Collaborate with other social and health partners to recommend care programs according...
