Position Overview:



The Behavioral Health Home Organization Nurse Care Manager (BHHO NCM) carries out medical functions to all clients receiving services from the BHHO team. Medical functions include basic psychiatric health assessments, treatment, monitor medications, education, and coordination of health care provided to clients in the community.
