Job Description

Engages and provides care and support to Behavioral Health clients, patients, and/or families. Performs psychosocial assessment and care management. Plans, implements,

and documents care and treatment. Collaborates with clinical and non-clinical team members and supports multidisciplinary team.

Job Responsibility

1.Assesses clients and conducts psychosocial assessment with family, significant others, and/or professional collaterals to determine patient conditions and needs.

2.Provides specialized therapeutic interventions, individual and/or group psychotherapy, as needed, as well as crisis counseling and intervention services, as appropriate.

3.Provides complex care management including identification and linkage to community services and resources, and/or access or transfer to a higher level of behavioral health care (e.g. partial hospitalization, inpatient hospitalization) as appropr...
