Qualifications:

• Current unencumbered license to practice as an LPN by the Louisiana State Board of Practical Nurse Examiners.
• Current BLS certification

Job Specific Duties:

• Assists in the admission of new residents: interviews resident and/or family member on admission, performs assessments, orients new resident to the facility, assists in the formulation of the initial care plan
• Reassesses residents at appropriate intervals – quarterly assessments as assigned. Communicates resident's progress through recording pertinent notes and reporting to oncoming shift.
