About The Role - Refine and develop integrated care models with a focus on transitional, pre-, and post-hospital workflows, to improve patient outcomes.
- Manage and analyse data to support decision-making.
- Operationalise new or developing models of health and care that integrate hospital and community.
- Perform data collection through administering surveys and data collation for department programmes.
- Engage and collaborate with healthcare partners in the community.
- Co-supervise and support the daily scheduling, charging and billing operations of the Community team.
- Additional duties as assigned by supervisor.
What We Are Looking For - Bachelor's degree in Healthcare Administration, Business Administration, or related field
