We are seeking a dedicated Afterhours RN Care Manager to provide high-quality, compassionate hospice care outside standard business hours. This role ensures seamless coverage, continuity of care, and strong clinical management for patients and families during nights, weekends, and on-call hours.



This role involves work 7 on- 7 off. Shift begins at 3pm and ends 8am the following day. You will cover on call starting at 5pm. Work 2 weeks/month and be paid for 4!



**About the Role**

The Afterhours Care Manager RN is responsible for assessing and coordinating patient and family needs during after-hours shifts and maintaining primary clinical oversight for assigned patients.
