Summary The Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) practices in Home Based Primary Care (HBPC) within the Geriatric Extended Service to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The NP provides direct Veteran care and care coordination, as it pertains to Veterans and their care within the HBPC Program. Responsibilities The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients.
