Salary Range: $60,000.00 To $75,000.00 Annually





Position Summary:

Responsible for clinical and financial evaluation of hospital and community referrals for admission and re-admission to St. John's Home, including all aspects of the process: screening, insurance verification and authorization, continuous communication with families & various community care partners, bed management, and oversight of the admission paperwork. Works in collaboration with other departments to provide support to residents and families, as well as maintain census and payer mix.
Qualifications & Education Required:

+ Bachelor’s Degree in Social Work or Human Services OR Bachelor’s Degree in Nursing or NYS RN Licensure OR Licensed Practical Nurse (LPN), with a minimum of three years’ experience in LTC with experience with the admissions process
