Summary:
Responsible for overseeing the referral and admission process for the Inpatient Rehabilitation Center in conjunction with unit leadership and the Medical Director

Responsibilities:
• Completes timely pre-admission assessment of candidates referred by acute care case managers, therapists, physicians, and external referral sources
• Assessments include a review of the medical record, family interviews, and consultation with unit leadership and the rehabilitation physician to determine appropriateness of admission
• Determines patient/family overall goal for rehabilitation, establishes a preliminary discharge plan and determines available insurance benefits for inpatient rehabilitation
