The Advanced Practice Provider (APP) will play a key role in care coordination, supporting smooth patient transitions across the continuum of care. This position involves active participation in daily and multidisciplinary rounds, early identification of discharge barriers, patient education, and collaboration with the care team to align goals and reduce delays. The APP will also leverage data to identify trends, implement targeted interventions, and utilize standardized protocols to promote consistency and reduce length of stay.
