About the role
We have an exciting opportunity for a motivated and experienced nurse to step into a leadership role as Associate Charge Nurse Manager (ACNM) Wellington Community Health team.
As ACNM, you'll play a key role in supporting acute flow and facilitating safe, timely discharges from hospital to home. You'll work closely with District Nurses, Enrolled Nurse, and Health Care Assistants, ensuring referrals are actioned efficiently, workloads are balanced, and high standards of care are maintained.
